Cancellation Policy

Immigration Medical Exam Booking Policy Immigration (USCIS I-693) medical exams are performed by appointment only. A completed intake form and valid government-issued photo identification are required at the time of your visit. The immigration medical exam is cash-pay only and is not billed to insurance. Payment is due at the time of service. Required laboratory testing and vaccinations are ordered by the physician but are completed through third-party laboratories and pharmacies. Patients may pay these providers directly using discounted cash rates or their insurance, depending on their preference and coverage. Please arrive on time for your appointment. Late arrivals may require rescheduling. If you need to cancel or reschedule, we request at least 24 hours’ notice to allow availability for other patients. Results are provided only after all required documentation, lab results, and vaccination records are complete. Completed I-693 forms are issued in a sealed envelope per USCIS requirements and must not be opened. Seattle Aviation Medical does not guarantee immigration outcomes or application approval. Our role is to complete the required medical evaluation accurately and in accordance with USCIS guidelines.
