Cancellation Policy

Coast Guard Medical Exam Policy (CG-719K) Seattle Aviation Medical provides Coast Guard CG-719K medical examinations for applicants seeking or renewing a Merchant Mariner Credential (MMC). All exams are performed in accordance with current United States Coast Guard medical standards. Appointments are required. We do not accept walk-ins. Please complete the medical history portion of the CG-719K form prior to your visit when possible and bring all required documentation. Payment Policy This is a flat-fee, cash-pay service. Payment is due at the time of scheduling or at the time of service, depending on booking selection. We do not bill insurance for occupational certification exams. The examination fee covers completion of the CG-719K form during the scheduled visit. Additional documentation requests, extended record review, letters, or follow-up paperwork beyond the standard form may incur additional fees. Medical Qualification Determinations Completion of the examination does not guarantee credential approval. The United States Coast Guard makes all final determinations regarding medical qualification and issuance of Merchant Mariner Credentials. If additional testing, specialist evaluation, or supporting medical records are required under Coast Guard standards, you will be informed during or after the visit. Costs associated with outside testing or specialist consultations are the responsibility of the applicant.
